![]() ![]() This study looks to expand on I-STAT use in the Emergency Department through troponin testing. Troponin refers to three different proteins that help regulate the contractions of the heart and skeletal muscles. iSTAT: Enzyme-Linked Immunosorbent Assay (ELISA) iSTAT. The analytical performance of one such assay, the i-STAT cardiac troponin assay, has been shown to be clinically usable in humans and is a precise and sensitive monitor of cTnI as a bedside test. The I-STAT is currently only utilized clinically in the Emergency Department for arterial blood gases due to respiratory therapy being the only personnel trained on I-STAT use. Troponin I: Exclusion diagnosis of acute myocardial infarction. The i-STAT cardiac troponin I (cTnI) test is an in vitro diagnostic test for the quantitative measurement of cardiac troponin I (cTnI) in whole blood or plasma. two in skeletal muscle and one in cardiac muscle. Troponin-I subunit exists in three isoforms. Troponin is the contraction regulating protein complex of striated muscle and consists of three distinct proteins: Troponin-I, Troponin-C, and Troponin-T. An Abbott iSTAT troponin-I (cTnI) test is reported as 600 ng/L which is abnormal (definition 40ng/L. Troponin testing utilizing the iSTAT will be used as a diagnostic test to look for acute changes over time (0,6 hours) as well as chronic elevations. The purpose of this study is to see if the I-STAT could be used clinically for the second troponin test in place of the current lab test, and whether this in turn would save time in diagnosing a heart attack or determining a patient's disposition in the Emergency Department. Performed at authorized locations by point of care operators. Description: A patient arrives in ED with chest pain. The Abbott I-STAT clinical analyzer is a hand-held device that is able to perform the troponin test in roughly 10 minutes instead of the hour that standard testing takes. Frequently, patients in the Emergency Department will have two troponin tests, one when they first enter the hospital and a second three hours later, as troponin levels take time to reach a detectable level in the blood after damage to the heart occurs. This particular test takes about an hour to run and return with a result. The i-STAT cTnI Test is an in vitro diagnostic test for the quantitative measurement of cardiac troponin I in heparinized whole blood or plasma samples. Patients in the Emergency Department undergoing evaluation for chest pain have a blood test called a troponin done as part of their evaluation. ![]()
